In May 2022 Park Senior Villas Goodyear, AZ received Notice of intent to revoke its licensure through the State of Arizona as it had been operating without a Assisted Living Manager through part of the Covid 19 Pandemic.
While State of Arizona is in part to blame due to “Phone it in” inspections, and lack of oversight by its Directors, this Senior Living in Goodyear /Assisted Living in our opinion; targeted families who were in distress and in need of assisted living options knowing they didn't have capability or staffing to care for them.
Park Senior Villas Goodyear, AZ didn't adhere to governing rules of ALTCS, Mercy Care, Arizona State Health Dept. let alone fill its ethical and legal obligation.

All 9 of Park Senior Villa's individually licensed buildings in Goodyear, AZ had finding which is verifiable through Arizona State Department of Health, which conducted an in-depth investigation of the facility which had been without a Licensed Assisted Living Manager for nearly a year. This was not mentioned on the tour of facility. The in-depth investigation was launched July and ended Nov 2022 with findings of which generated penalty by the State, which fell short as their enforcement parameters for penalties are limited of which few can be levied regarding mistreatment, hiring shortfalls, and bad business practices. Rather it is left to the families and those in care to file against this company in civil court.
Minor monetary penalties were levied against this assisted /senior living (Goodyear Arizona) for absent and expired online CPR Certifications, missing or non existent fingerprint clearance cards, scheduling shortfalls, missing medications, lack of enrichment, lack of nourishment, forged signatures/ documents relating to residents care which is a Felony, accepting Mental Patients and housing them with vulnerable adults without a licence for mentally disturbed, medication mismanagement (just naming a few here) but those infractions and sanctions surely don't go far enough to penalize the company or its management, nor does it compensate those residence who were entrusted in their care. A gross injustice.
Park Senior Villa's Goodyear, AZ fostered an environment where residents who were in need of in direct care services were found left alone without assistance overnights during 2022 and at times left without a manager on its grounds. At times Park Senior Villas has assistant care givers alone. Loved ones were led to believe each and every building had 24 hour on site care. The state of Arizona, and Mercy Care were also led to believe as well.

Direct from AZ Dept Health
Based on documentation review, record review, and interview, the manager failed to ensure a policy and procedure was developed, documented, and implemented to establish a process for checking on a resident receiving directed care services during nighttime hours to ensure the resident's health and safety.
Findings include:
1. The facility was authorized to provide directed care services.
2. Review of R1's, R2's, R3's, and R4's records revealed service plans indicating the residents' received directed care services.
3. The surveyors requested to review the facility's policies and procedures for establishing a process for checking on a resident receiving directed care services during nighttime hours to ensure the resident's health and safety. E1 presented the surveyors with a document titled "Check list of Nightly Duties".
1. Review of the personnel schedule for August 2022 revealed no caregiver was scheduled to work the 7pm - 10pm shift August 5th and no caregiver was scheduled to work the 2pm - 10pm shift August 6th.
2. During an interview, E1 reported the resident care coordinator filled in when needed. E1 acknowledged documentation was not maintained of the caregivers and assistant caregivers working each day, including the hours worked by each.

Based on documentation review, record review, and interview, the governing authority failed to ensure compliance with A.R.S. § 36-411, for one of eight employees reviewed. The deficient practice posed a risk if E5 was a danger to a vulnerable population.
Findings include:
1. A.R.S. § 36-411 states, "A... as a condition of licensure or continued licensure of a residential care institution...
2. Review of E5's personnel record revealed a hire date of May 11, 2015. A fingerprint clearance card was available for E5 that listed the expiration date of the card as May 26, 2021.
3. Review of the DPS fingerprint clearance card database revealed E5's fingerprint clearance card was "invalid".
4. During an interview, O1, a DPS representative, reported E5's fingerprint card was "expired and not valid".
5. During an interview, E1 reported E5 did not currently have a fingerprint card.

Based on observation and interview, the manager failed to ensure the premises were cleaned.
Findings include:
1. During the facility tour with E2, the surveyors observed R2's bedroom. R2's bedroom had a strong urine odor and dried urine stains on the floor.
2. In an interview, E2 reported the dried liquid on the floor was urine. E2 reported R2 urinates frequently throughout the night on the floor, walls, and closets. E2 acknowledged R2's bedroom was not clean.
Family Members are tired of lack of accountability from Arizona Health Dept, State Governments, and the Gaps left in Federal regulation which provides incentives to foster maltreatment of Elderly.
Senior Living companies are a liberty to manage and create content for Facebook aka Meta, Instagram, Twitter,and other platforms, flyers, commercials, marketing materials, and hire for profit placement assistance services without fear of repercussions as they go unchecked and any business can create any story in order to get your money.
We live in world where telling the truth is cancelled, modified, and skewed for the benefit of "senior living" communities stakeholders. It is obscene. It needs rectified.

NEARLY ALL...
Review sites such as Google, or Yahoo, MSN, and even Senior Living Placement Services for elderly are edited and manipulated in order to portray what their clients (the business owner) wants you to believe and they sell that to you the consumer as free assistance while youre under duress.
Companies pay to remove the real reviews, and pay others to edit site content omitting these reviews (Caring.com, and Just for Mom). These "FREE" placement companies get a finders fee if they refer a resident to the tune of one month's rent sometimes more.
Folks this is market manipulation at its finest and Park Senior Villa's have deployed these tactics.

The federal Lanham Act allows civil lawsuits for false advertising that “misrepresents the nature, characteristics, qualities, or geographic origin” of goods or services. 15 U.S.C. § 1125(a). The FTC also enforces false advertising laws on behalf of consumers.

Forgery Laws in Arizona
In Arizona a person may be found guilty of committing forgery in violation of A.R.S. 13-2002 if they create, complete, or possess, offer, or present information that contains false information, false documents, or instruments with the intent to defraud.

Charges of forgery are categorized as a Class 4 felony., which expose a person to prison terms of 1.5 to 3.75 in prison, for non-dangerous forgery offenses.
